DEXA Scans for Bone Health

by OB/GYN | Oct 21, 2025 | Women's Health

Bone health often gets overlooked—until a problem arises. As women age, declining estrogen levels can weaken bones, increasing the risk of osteoporosis and fractures. A simple, noninvasive DEXA scan can help detect bone loss early, long before symptoms appear.
At Cary OB/GYN in North Carolina, we help women protect their bone health through preventive screenings, personalized care, and treatments such as hormone replacement therapy (HRT) when appropriate.

What Is a DEXA Scan?

A DEXA scan (Dual-Energy X-ray Absorptiometry) measures bone mineral density (BMD). It’s the most accurate test available for diagnosing osteoporosis and assessing fracture risk.
The scan is quick, painless, and uses low-dose X-rays to measure the strength of bones—most commonly in the spine and hips, where fractures are most likely to occur.

During the test, you’ll lie flat on a table while a scanning arm passes slowly over your body. The process typically takes less than 15 minutes, and results are available almost immediately.

When Should Women Get a DEXA Scan?

While everyone’s health needs differ, most experts recommend that women begin DEXA screening at the following times:

  • Age 65 or older: All postmenopausal women should have at least one DEXA scan.

  • Younger than 65 with risk factors: Screening may begin earlier if you have:

    • Early menopause (before age 45)

    • Family history of osteoporosis or fractures

    • Low body weight or history of eating disorders

    • Previous fractures or long-term corticosteroid use

    • Certain medical conditions (like thyroid disorders or rheumatoid arthritis)

Your provider may also recommend repeat scans every 1–2 years to monitor bone density changes over time.

Why Bone Health Declines After Menopause

Estrogen plays a critical role in maintaining strong, healthy bones. It helps regulate bone turnover—the process in which old bone is replaced with new tissue.
After menopause, estrogen levels drop sharply, and bone loss accelerates. Women can lose up to 20% of their bone density in the first five to seven years after menopause, significantly increasing the risk of osteoporosis and fractures.

The Role of Hormone Replacement Therapy (HRT) in Bone Health

Hormone Replacement Therapy (HRT) can help slow or even prevent postmenopausal bone loss. By restoring estrogen levels, HRT reduces the rate at which bone tissue breaks down and improves calcium absorption.

Benefits of HRT for Bone Health

  • Helps maintain or increase bone mineral density

  • Reduces the risk of hip and spinal fractures

  • Provides added relief from menopause symptoms such as hot flashes, sleep issues, and mood changes

Who Should Consider HRT

HRT may be an option for:

  • Women under 60 or within 10 years of menopause

  • Those with significant menopausal symptoms

  • Women at increased risk of osteoporosis without contraindications (such as certain cancers, blood clots, or liver disease)

Your OB/GYN will review your medical history and risk factors to determine if HRT—or other bone-protective treatments like bisphosphonates or selective estrogen receptor modulators (SERMs)—is best for you.

Other Ways to Support Bone Strength

Even if you’re not a candidate for HRT, lifestyle changes can have a major impact on bone health:

  • Calcium and Vitamin D: Essential nutrients for bone formation and strength.

  • Weight-bearing exercise: Walking, strength training, and yoga stimulate bone growth.

  • Quit smoking and limit alcohol: Both can accelerate bone loss.

  • Regular checkups: Ensure your bone density is monitored and your care plan is adjusted as needed.

Protecting Your Bones for Life

Osteoporosis often develops silently, but it’s preventable and manageable with early care. A DEXA scan is a simple, effective first step to understanding your bone health.

At Cary OB/GYN, our team provides expert guidance in bone health, menopause management, and personalized hormone therapy to help you stay strong and active at every stage of life.
